App Feedback Survey - Template

How's it going with your app? A survey is a great way to find out!  We recommend surveying your app followers at least once a year to gain insight into usage and new areas of need.

The following survey template is formatted so it can be copied directly into Google Forms (each question clearly labeled with suggested question type).
